Our Behavioral Health Home (BHH) Care Coordination program is designed to give children and youth the support they need to truly thrive. With BHH, every child receives a dedicated care coordination team that partners closely with your family to understand your unique needs, strengths, and challenges. Together, we will create a personalized action plan that helps your child and your family reach your goals, ensuring you have access to the right resources at the right time.

BHH services are available in Androscoggin, Oxford, Franklin, York, and Cumberland Counties.

Does Your Child Qualify?

Children ages 0 to 21 with a behavioral health diagnosis, Autism, or an intellectual or developmental diagnosis within the past year may qualify. Full MaineCare eligibility is required. Your child should also be experiencing challenges in school, at home, or in the community. If needed, we can assist in setting up assessments to determine eligibility.

Your Child’s BHH Care Coordination Team

    BHH Care Coordinator: Your Care Coordinator is your family’s guide and advocate. They are committed to truly getting to know your child and building a plan that reduces stress, empowers your family, and opens doors in school and the community.

    • Takes time to understand your child’s strengths, challenges, and existing supports.
    • Works with your family to develop solutions that fill gaps and help your child thrive.
    • Connects you with the caregivers and resources that fit your child’s journey.
    • Advocates for your child’s needs across school, community, and basic needs.
    • Provides regular check-ins and adjusts the plan as your child grows.

    Nurse Care Manager: Your Nurse Care Manager brings medical expertise and compassion to your child’s care team.

    • Conducts health screenings and develops personalized care plans.
    • Communicates directly with primary and specialty care providers.
    • Offers guidance for managing health concerns as they arise.
    • Takes proactive steps to prevent hospitalization.
    • Supports smooth transitions following any medical discharge.
    • Clinical Team Lead: The Clinical Team Lead monitors and guides your child’s support team, ensuring they receive high-quality, trauma-informed care. The Clinical Team Lead supervises the client team and monitors services to ensure consistent quality for youth.
    • Consulting Team: Your child’s progress is periodically reviewed by consulting primary care and psychiatric practitioners, so their needs are always front and center, and nothing slips through the cracks.
    • Family Peer Support Specialist: (Optional) If you choose, a Family Peer Support Specialist joins your team, bringing insight and encouragement from someone who has navigated similar experiences. They help families navigate complex systems, including school and healthcare, strengthen caregiver advocacy skills, and promote active family involvement in decision-making.
    • Youth Peer Support Specialist: (Optional) Youth Peer Support empowers your child to build confidence and community with someone who genuinely understands their journey. They build supportive, strength-based relationships with youth, provide one-on-one peer support, help youth raise their voice in their own services, and promote youth development and empowerment.
