Professional Liability Insurance must additionally meet the following requirements:
- The policy must list the provider’s name as the insured.
- Individuals who submit a document listing only a business name will be asked to provide an alternate policy that includes their own name as the insured party.
- The policy must be at least 1 million per claim and 3 million per aggregate.
- The file should be in a pdf format versus image file.
