Dedicated support, resources, and strength for Maine’s veterans and families
Many veterans don’t know all the benefits and supports available to them — and families often carry that weight too. At Health Affiliates Maine, we’re here to walk alongside you so you don’t have to figure it out alone.
Our case management services are free, available statewide, and made possible through grant funding from the Veteran’s Administration Staff Sergeant Parker Gordon Fox Suicide Prevention Grant Program and the Maine Office of Behavioral Health.

What to Expect
- Start with a call. After you complete the referral form or call us directly, our Program Coordinator will reach out within seven business days.
- A no-pressure first step. That first call is a conversation, not a commitment. We’ll listen, answer your questions, and explore what supports might fit best.
- Case manager connection. If you’re interested in moving forward, you’ll be paired with a case manager who will work with you to create a plan of care.

- Ongoing support. Together, you’ll decide how often to meet and in what way — phone, telehealth, in person, or a mix. Support can be short- or long-term, depending on your needs.
- Extra help when needed. If you’re experiencing significant stressors, our Program Coordinator may also stay involved for additional support.

Fox vs. Non-Fox Case Management
Health Affiliates Maine participates in the Staff Sergeant Parker Gordon Fox Suicide Prevention Grant Program (SSG Fox SPGP).
Non-Fox Case Management
Veterans are paired with a dedicated case manager who helps set goals, connect to services, and provide ongoing support, while also managing other clients.
Fox Case Management (SSG Fox SPGP)
In the Fox program, veterans still have a dedicated case manager, but they also gain “top cover” support from coordinators and a program team who can step in whenever extra help is needed. This means:
- More consistent, timely, and proactive support
- A stronger safety net for the veteran
- Case managers with backup so they can serve more effectively
If you’re not eligible: You’ll still be connected to the services and support you need. There are no wrong doors — every veteran will be guided to help, no matter what.

But I’m not suicidal — is this for me?
Yes. Suicide prevention isn’t only about people in immediate crisis. It’s about early support, reducing stress, and staying connected. That might mean help with housing, benefits, employment, peer support, or mental health care. Think of it like preventative health care: these services help veterans and families stay strong before challenges become overwhelming.
Do I need insurance?
No. Veterans’ case management is provided at no cost, with no obligation, and no health insurance required.
Will this affect my VA benefits?
Not at all. Our services are designed to complement your VA benefits, not interfere with them
Is this confidential?
Yes. Everything you share is confidential. Your information will only be used to connect you with services and support.
What exactly is case management?
Case management means having someone in your corner who helps you navigate life’s stressors. That might mean finding housing, connecting with healthcare, locating community resources, or just having someone to talk through challenges with.
How long will I work with a case manager?
That depends on you. Some veterans work with us for a short time until a need is met, while others choose longer-term support. You and your case manager decide together what works best.
What does it mean to participate in the Fox Program?
You’ll still have your dedicated case manager, but with Fox, you also gain a coordinated team that provides backup. That means faster responses, broader support, and the reassurance that you’re never navigating this alone.
What qualifies me for the Fox Program?
- Having served in the active military, naval, or air service (under qualifying discharge conditions)
- Meeting eligibility under specific U.S.C. sections (including certain combat service and discharge statuses)
- Having one or more risk factors, such as mental health challenges, substance use, chronic health conditions, or past trauma
- Completing baseline wellness and risk screenings (ages 18+)
What’s required of me if I join Fox?
Participation means staying engaged — completing assessments, attending check-ins, and working with your case manager on your plan. You’ll also be asked to share feedback before wrapping up, so we can keep improving the program.
